Accutane Purge vs Breakout — What’s Normal and What’s Not

Month 1–2 is hell. Here’s how to tell if your skin is purging or if something’s actually wrong.

You started Accutane expecting clear skin. Instead, month 1 brought the worst breakout of your life. Your face is worse than before you started. You’re panicking. Here’s the thing: this is probably normal. The Accutane purge is real, it’s temporary, and understanding it will stop you from quitting the one treatment that actually works.

What the purge actually is

Isotretinoin accelerates skin cell turnover dramatically. All the microcomedones — tiny clogged pores that haven’t surfaced yet — get pushed to the surface at once. These are breakouts that were going to happen over the next 6–12 months, compressed into 2–6 weeks. It looks worse because it IS worse, temporarily. But it’s your skin flushing out the backlog.

Purge vs actual breakout: how to tell

THE PURGE: Appears in your usual breakout zones (where you normally get acne). Consists of whiteheads, small pustules, and comedones surfacing. Starts within the first 2–4 weeks. Each individual spot heals faster than your pre-Accutane breakouts. Gradually improves week over week, even if slowly.

A REAL PROBLEM: Appears in new areas you’ve never broken out before. Includes deep, painful cysts that weren’t there pre-treatment. Gets progressively worse after week 6 with no improvement. Accompanied by signs of allergic reaction (widespread rash, hives, swelling). If this describes you, contact your dermatologist. Your dose may need adjusting.

Insight

The purge typically peaks around weeks 3–5 and resolves by weeks 6–8. If you’re still getting significantly worse after 8 weeks with zero improvement, that’s worth a conversation with your derm.

How to manage the purge

You cannot prevent the purge. But you can minimize scarring and discomfort. Rule #1: do not pick, squeeze, or extract. Your skin is thinned by Accutane and will scar much more easily than normal. Rule #2: anti-inflammatory ingredients are your friend. SKIN1004 Centella Ampoule ($18) calms redness and speeds healing of individual spots without any actives that would irritate Accutane skin.

The healing support

COSRX Snail 92 All-in-One Cream ($15) is thicker than the essence version and works as both a soothing treatment and a moisturizer. The snail mucin’s natural allantoin and glycoproteins support wound healing — exactly what purge spots need. Apply a generous layer at night over your usual routine. The spots will flatten and fade faster.

The timeline most people experience

Weeks 1–2: Dryness starts. Existing acne may seem unchanged. Weeks 2–4: The purge begins. New spots surface rapidly. This is the hardest part mentally. Weeks 4–6: Purge peaks then starts to slow. Individual spots heal faster. Weeks 6–8: Noticeably fewer new spots. Skin starts clearing. Months 3–4: Significant clearing. Most people see 60–80% improvement. Months 5–6: Near-complete clearing for most patients. The finish line.

Every person who’s been through Accutane will tell you the same thing: the purge is temporary, the results are permanent. Don’t quit during the hardest part. Support your skin, trust the process, and let the isotretinoin do its job.
